MONITOR: The combined use of alendronate with naproxen may synergistically increase the potential for gastrointestinal (GI) toxicity, including severe gastric mucosal damage and ulceration. A blinded, randomized, crossover study with healthy volunteers (n=26) demonstrated a significantly higher incidence of gastric ulcers (38%) in subjects receiving the combination for 10 days, compared to subjects receiving naproxen alone (12%) or alendronate alone (8%). This effect is also possible when alendronate is given concomitantly with other nonsteroidal anti-inflammatory drugs (NSAIDs).
MANAGEMENT: Until more information is available, caution is advised if alendronate is used with other NSAIDs. During concomitant therapy, patients should be closely monitored for symptoms of gastric ulcers and should be advised to immediately report signs and symptoms such as severe abdominal pain, nausea, loss of appetite, diarrhea, dizziness, lightheadedness, or the appearance of black, tarry stools.
