Applies to:meperidine/promethazine and Fiorinal (aspirin/butalbital/caffeine)
MONITOR: Barbiturates may increase the nervous system depressant effects of meperidine. This action appears to be additive. Barbiturates may also increase the formation of the toxic metabolite normeperidine by inducing hepatic metabolism, which can cause generalized CNS stimulation, including seizures.
MANAGEMENT: Patients should be monitored for altered effects when receiving this combination. Dose adjustments may be necessary.
promethazine ↔ butalbital
Applies to:meperidine/promethazine and Fiorinal (aspirin/butalbital/caffeine)
The therapeutic effects of phenothiazines and the plasma levels of barbiturates may be reduced when these drugs are used together. The mechanism may relate to induction of CYP450 hepatic metabolism by barbiturates. The clinical significance is unknown. Patients should be monitored for altered effects of either agent. Tailoring of phenothiazine dosage may be necessary.
