MONITOR: Coadministration with carbamazepine may decrease the plasma concentrations of risperidone and its pharmacologically active metabolite, 9-hydroxyrisperidone. The proposed mechanism is carbamazepine induction of CYP450 3A4-mediated metabolism. In 11 patients comedicated with carbamazepine, the steady-state plasma concentrations of risperidone and 9-hydroxyrisperidone were lower compared to those of 23 patients who were treated with risperidone alone, although the difference was statistically significant only for the metabolite. In a subgroup of five patients who were assessed with and without carbamazepine comedication, dose-normalized plasma risperidone and 9-hydroxyrisperidone concentrations were significantly lower for combination therapy than for risperidone alone. In a case report, the interaction was suspected in two patients treated with the drugs who developed parkinsonian symptoms several weeks following the discontinuation of carbamazepine. The symptoms resolved completely in both cases within a week after risperidone dosage was reduced.

MANAGEMENT: Patients treated concomitantly with carbamazepine may require a higher dosage of risperidone to control psychotic symptoms. Pharmacologic response and plasma risperidone levels should be monitored more closely whenever carbamazepine is added to or withdrawn from therapy in patients stabilized on their existing antipsychotic regimen, and the risperidone dosage adjusted as necessary.
