MONITOR: Case reports suggest that the combination of lithium and clozapine may result in the development of neurologic toxicity, including neuroleptic malignant syndrome, tremors, spasms, gait abnormalities, involuntary muscle movements, seizures, delirium. The mechanism is unknown but may be related to additive serotonergic effects. Controlled studies are needed to determine causality of this drug interaction. Additionally, two cases of diabetic ketoacidosis have been reported in patients with family histories of diabetes after clozapine was added to a lithium regimen.
MANAGEMENT: Patients receiving this combination should be monitored for neurologic symptoms. Close attention should be paid to symptoms such as stiffness, tachycardia, hyperpyrexia, and diaphoresis if these drugs must be used together. In addition, routine blood glucose monitoring has been suggested.
