Interactions between your selected drugs
clarithromycin ⇔ conivaptan
Applies to: clarithromycin and conivaptan
CONTRAINDICATED: Coadministration with potent inhibitors of CYP450 3A4 may significantly increase the plasma concentrations of vasopressin receptor antagonists conivaptan and tolvaptan, which are primarily metabolized by the isoenzyme. The interaction has been studied with ketoconazole, a potent CYP450 3A4 inhibitor. According to the product labeling, administration of oral conivaptan 10 mg in combination with ketoconazole 200 mg resulted in 4- and 11-fold increases in conivaptan peak plasma concentration (Cmax) and systemic exposure, respectively, compared to administration alone. Similarly, coadministration with ketoconazole 200 mg increased tolvaptan AUC by 5-fold. The consequences of increased plasma concentrations of conivaptan and tolvaptan are unknown, and data are insufficient to support specific dosage adjustments for these drugs when used with potent CYP450 3A4 inhibitors.
MANAGEMENT: The use of conivaptan or tolvaptan with potent CYP450 3A4 inhibitors such as itraconazole, ketoconazole, voriconazole, nefazodone, delavirdine, protease inhibitors, and ketolide and certain macrolide antibiotics is considered contraindicated.
