ADJUST DOSING INTERVAL: Iron preparations may decrease the plasma concentrations of mycophenolic acid when coadministered orally. The mechanism is most likely due to formation of a drug-iron complex in the gastrointestinal tract, resulting in impaired absorption of mycophenolic acid. In a randomized, crossover study consisting of seven healthy subjects, a single dose of sustained-release iron (given as two 525 mg tablets of ferrous sulfate, equivalent to 210 mg of elemental iron) decreased the mean peak plasma concentration (Cmax) and area under the concentration-time curve (AUC) of mycophenolic acid (from four 250 mg capsules of mycophenolate mofetil) by 94% and 91%, respectively, compared to administration of mycophenolate mofetil alone.

MANAGEMENT: Patients requiring concomitant therapy with oral mycophenolic acid and iron or iron-containing supplements should separate the administration times of the medications by at least two hours. In addition, immediate-release iron products may be preferable to sustained-release formulations to minimize the likelihood of an interaction.
