MONITOR: Diltiazem has been reported to increase the effects of quinidine. In healthy volunteers the area under the concentration-time curve of quinidine was increased by 51% and the half-life was prolonged from 6.3 to 9.3 hours. The mechanism of action is unknown, but may be related to inhibition of CYP450 3A4 hepatic metabolism. QT prolongation has been reported. However, data have been conflicting: another study reported no interaction between diltiazem and quinidine.
MANAGEMENT: Close monitoring of quinidine levels, pharmacological effects, and ECG, and QT interval is recommended if diltiazem is added to or withdrawn from therapy. Alternatively, felodipine and amlodipine do not inhibit CYP450 3A4 and are not expected to affect quinidine levels. Patients should be advised to notify their physician if they experience tinnitus, hearing loss, visual disturbances, diarrhea, dizziness, weakness, headache, syncope, or palpitations.
