GENERALLY AVOID: Coadministration with the combination of calcium carbonate and magnesium hydroxide may reduce the therapeutic effects of pancrelipase in the treatment of steatorrhea. The mechanism involves precipitation of glycine-conjugated bile acids and formation of calcium and magnesium fatty acid soaps, resulting in a decrease in fat absorption. In addition, by increasing gastric pH, antacids may reduce the resistance of the enteric coating of some formulations of pancreatic enzymes, resulting in earlier release and destruction of enzymatic activity.
MANAGEMENT: Patients receiving pancrelipase for steatorrhea should avoid taking the medication together with a combination of calcium carbonate and magnesium hydroxide. Calcium carbonate and magnesium hydroxide may be found in certain antacids, and calcium carbonate may also be found in certain calcium or mineral supplements.
