Some salicylates may impair the tubular secretion of canrenone, the main active metabolite of spironolactone. This effect could inhibit the natriuretic properties of spironolactone. Data are available for aspirin. If diuresis is inadequate, the clinician should consider discontinuing the salicylate or increasing spironolactone dosage while paying careful attention to the patient's serum potassium concentration.
