ADJUST DOSE: Coadministration with carbamazepine may significantly decrease the plasma concentrations of aripiprazole. The mechanism is carbamazepine induction of CYP450 3A4, the isoenzyme that is partially responsible for the metabolic clearance of aripiprazole. According to the manufacturer, carbamazepine (200 mg twice a day) decreased the peak plasma concentration (Cmax) and area under the concentration-time curve (AUC) of both aripiprazole (30 mg once a day) and its active metabolite, dehydro-aripiprazole, by approximately 70%.
MANAGEMENT: Pharmacologic response to aripiprazole should be monitored more closely whenever carbamazepine is added to or withdrawn from therapy. The manufacturer recommends that aripiprazole dosage be doubled when carbamazepine is added to therapy, and additional dosage increases be based on clinical evaluation. Aripiprazole dosage should be reduced accordingly if carbamazepine is discontinued.
