Usual Adult Dose for Congestive Heart Failure
Initial dose: 1 tablet of (enalapril-hydrochlorothiazide 5 mg/12.5 mg) once a day. Determine dosage by individual titration.
Maintenance dose: May increase up to a maximum dose of 2 tablets of (enalapril-hydrochlorothiazide 10 mg/25 mg) in 1-2 divided doses a day.

Renal Dose Adjustments
The usual regimens of therapy with enalapril-hydrochlorothiazide need not be adjusted as long as the patient's creatinine clearance is >30 mL/min. In patients with more severe renal impairment, loop diuretics are preferred to thiazides, so enalapril-hydrochlorothiazide is not recommended.
Liver Dose Adjustments
Thiazide diuretics should be used with caution in patients with hepatic impairment.
Dose Adjustments
The hydrochlorothiazide dose should generally not be increased until 2-3 weeks have elapsed.
Precautions
Safety and effectiveness have not been established in pediatric patients (less than 18 years of age).
Dialysis
Enalapril is moderately dialyzable (20%-50%).
