Usual Adult Dose for Congestive Heart Failure

Initial dose: 1 tablet of (captopril-hydrochlorothiazide 25 mg/15 mg) orally once a day, one hour before meals. Determine dosage by individual titration.
Maintenance dose: May use additional captopril or hydrochlorothiazide as individual components or change to an increased strength of the combination tablet. May give the combination tablet in divided doses up to 3 times a day. In general, daily doses of captopril should not exceed 150 mg and of hydrochlorothiazide should not exceed 50 mg.

Usual Adult Dose for Hypertension

Initial dose: 1 tablet of (captopril-hydrochlorothiazide 25 mg/15 mg) orally once a day, one hour before meals. Determine dosage by individual titration.

Renal Dose Adjustments

Hydrochlorothiazide is generally not effective in patients with creatine clearance less than 25 mL/min. Captopril-hydrochlorothiazide should be used with caution in patients with severe renal disease.

Liver Dose Adjustments

Thiazide diuretics should be used with caution in patients with hepatic impairment.

Dose Adjustments

Dosage adjustments should generally be made at 6 week intervals, unless the clinical situation demands more rapid adjustment.

Precautions

There is limited experience reported in the literature with the use of captopril in the pediatric population; dosage, on a weight basis, was generally reported to be comparable to or less than that used in adults. The manufacturer states that safety and effectiveness of captopril-hydrochlorothiazide have not been established in pediatric patients (less than 18 years of age).

Dialysis

Captopril is moderately hemodialyzable (20%-50%).
Captopril is not removed by peritoneal dialysis.
