Usual Adult Dose for Hypertension
Initial dose: 1 tablet (16 mg-12.5 mg) orally once a day.
Maintenance dose: Once or twice daily administration with total candesartan daily doses ranging from 8 mg to 32 mg.
Renal Dose Adjustments
Hydrochlorothiazide is not recommended in patients with creatinine clearance less than 25 mL/min because it is not expected to be filtered into the renal tubule. Thiazides should be used with caution in patients with severe renal disease.
Liver Dose Adjustments
Thiazides should be used with caution in patients with impaired hepatic function.
Precautions
Safety and effectiveness have not been established in pediatric patients (less than 18 years of age).
Dialysis
Candesartan is not removed by hemodialysis. An increased risk of hypotension exists if the dose is given immediately following dialysis. Ideally, the drug should be given when the patient is hemodynamically stable. Data is not available regarding the dialysis clearance of hydrochlorothiazide.
Other Comments
Candesartan-hydrochlorothiazide may be given without regard to meals.
