Fexofenadine is primarily eliminated unchanged by the kidney. Patients with renal impairment may be at greater risk for adverse effects from fexofenadine due to decreased drug clearance. The manufacturer recommends an initial dosage of 60 mg once daily in patients with renal dysfunction (CrCl < 80 mL/min).
