I have really bad heartburn. I was on nexium and it worked like a dream but my insurance stopped and I have to use otc meds. Is there a difference between the one with magnesium and the one without? Will magnesium alone help?

No, magnesium alone will not help your heart burn.
The OTC version of omeprazole is ompeprazole magnesium (the magnesium salt of omeprazole - the difference is inconsequential) - a delayed release version of omeprazole.
They are not bioequivalent - meaning you can not swap one for the other and expect the same effect.
