Why do I have vaginal bleeding when I urinate?

I have had a hysteroscopy, several uterine byopsies, several ultrasounds by stomach and vagina, a polyp partially removed during my hysteroscopy and a pap smear. All tests were negative for cancer. Again, I have been taking Coumadin for about 2 years.
