Information on new FDA warning is not clear and seems not to indicate that it is based on only 4 cases ever reported in the US by the NAAED. I want to know if these are the same as doses for the prevention of migraine, not epilepsy.

Just went through the FDA warning. As you said, there was no dose. The warning however, was applied to topamax as a whole. Therefore, caution should be used in all doses.
It also mentioned a risk vs benefits comparison. Therefore, you have to look at it from overall benefits. In my opion, the benefit of preventing migrans is lower than the benefit of siezure prevention.
Since there are low benefits and potential risks I would recommend against the use.

Aren't these types of issues dose related?
To be honest I cannot comment on that - as I stated there is a risk/benefit. There was a good quote from one of the conductors of the study that said, "--although the study included only epilepsy patients, the results may be of particular relevance for migraine patients "since [Topamax] is also used for preventing migraines, which is an even more common condition that also occurs frequently in women of childbearing age."

The only risks I could find were for a cleft palate or a cleft lip. Both of which can be corrected with surgery... I'm going to try to find the website again and come back and repost it here for you.
