I would reiterate that you still realize significant pain prior to the end of the 12 hour period. I had the exact same issue and I explained this to my Pain Management Physician (Neurologist) I told him I preferred not to increase the dosage (more of the narcotic) so instead, we shortened the periods of doseage.
