Arthritis in Children Linked to Infections

TUESDAY, May 1 – Children with juvenile arthritis have higher rates of bacterial infection when hospitalized than children without arthritis, a new study says. While taking high-dose steroids was associated with a higher infection risk among kids with arthritis, other arthritis drugs – methotrexate and tumor necrosis factor alpha (TNF) inhibitors – were not. Researchers analyzed Medicaid data on nearly 8,500 children with juvenile idiopathic arthritis and more than 360,000 arthritis-free children. Idiopathic means that a disease occurs without a known cause. Pharmacy claims were examined to determine arthritis patients' use of drugs that suppress the immune system, including high-dose glucocorticoids (steroids), methotrexate and TNF inhibitors, according to the study published May 1 in the journal Arthritis & Rheumatism. Long-Term Methotrexate Won't Help Kids With Juvenile Arthritis

TUESDAY, April 6 – Longer treatment with the drug methotrexate doesn't reduce relapse rates in patients with juvenile idiopathic arthritis (JIA) that has gone into remission, a new study finds. The research included 364 JIA patients, median age 11 years, in 29 countries. They were randomly assigned to continue taking methotrexate for either six (group 1) or 12 months (group 2) after the beginning of disease remission, which meant that the children were temporarily free of persistent joint pain, swelling and stiffness. The median relapse-free time was 21 months in group 1 and 23 months in group 2. Of the 297 patients who stopped taking methotrexate while in remission, about 40 percent in both groups 1 and 2 had a JIA flare within a year. Fewer Childhood Deaths From Rheumatic Disease

MONDAY, Feb. 1 – Death rates for U.S. children with rheumatic diseases are much lower than previously reported, a new study has found. Cleveland Clinic researchers analyzed data on 48,885 patients in the national Pediatric Rheumatology Disease Registry (PRDR), who were diagnosed with rheumatic diseases between 1992 and 2001. There were 110 deaths among these patients, a number that was significantly less than expected, the researchers noted. The risk of death was notably greater for children with systemic lupus erythematosus and dermatomyositis, but not for systemic juvenile rheumatoid arthritis. The risk of death was markedly less for children with pain syndromes. Most of the deceased with inflammatory disease died of their disease or disease complications, while many of the deceased with pain syndromes died of non-natural causes, according to the study published in the February issue ... Etanercept, Methotrexate Safe for Juvenile RA

MONDAY, Oct. 27 – The drugs etanercept and methotrexate are safe and effective for long-term, continuous treatment of juvenile rheumatoid arthritis, new research finds. In a three-year study that enrolled about 600 patients, aged 2 to 18, some of the children took methotrexate alone, others took etanercept alone, and another group took both drugs. Etanercept was given as either a 0.4 mg/kg injection twice weekly or 0.8 mg/kg weekly, and methotrexate was given as dose greater than or equal to 10mg/m2 once a week. The researchers assessed the safety of the treatments by monitoring the occurrence of serious side effects, such as medically important infections and death. They determined the overall effectiveness of the medications by assessing the patients' joints and overall health.
