I guess he wasn't on 32 mg Of Suboxone long right? Reduce the dose asap. He's getting very messed up with all the opiates and Sub causes a person to need a lot pf other opiates to just to stay out of wds.

I think you should concentrate on switching him to just OxyContin for now. I'm not familiar with going from Sub to Methadone, But I do have experience with changing him to Oxy. Based on the Narcotic Equivalence Converter 24 mg of Sub would need 300 - 600 mg of OxyContin to keep him out of wds. In my experience you usually have to go with the top #. And if I base it on 32 mg of Sub the answer become 400 - 800 mg of OxyContin. So lower his Sub dose asap. Keep him off Sub at least 24 hours. That shouldn't be hard because Sub lasts 2 days before wds set in. So try and keep him off all opiates 48 hours. At that time start him on 400 mg of OxyContin. That may be enough. But you may have to go up to 800 mg of OxyContin. I'd go up by 80 mg tabs, since I think that's the largest it comes in. I am very hesitant to start him back on Methadone. I'd want to wait 1 month before giving him Methadone, and even then I would avoid it. I'd stick to short acting opiates for 1 month if it was up to me. What other drugs does he react well to besides Oxycodone? What about Dilaudid or Opana? Since OxyContin is a long term time released pill, can you use Dilaudid or Opana for BT pain.
