Mary, most times it on the box or insert. and they ahve a phone number. If they can find that just call and ask, if they do they will send a form to fill out. Thats what I did with Lexapro. Von-1
Thank you Mary. I did find their drug assistance program , but Prevacid is not listed as one of the drugs they will help with.
