I have been taking Arimidex for 3 years. For the last 6 months I have been treated for recurring yeast infections, all to no avail. Yesterday, more test results indicated 'no yeast' - best guess now is 'lichen sclerosis'.
Is this a common side effect?

Hello,
Very mild lichen sclerosus of the genital area may cause itching, but often causes no symptoms at all. If the disease worsens, itching is the most common symptom. Rarely, lichen sclerosus of the vulva may cause extreme itching that interferes with sleep and daily activities. Rubbing or scratching to relieve the itching can create painful sores and bruising, so that many women must avoid sexual intercourse, tight clothing, tampons, riding bicycles, and other common activities that involve pressure or friction. Urination can be accompanied by burning or pain, and bleeding can occur, especially during intercourse. When lichen sclerosus develops around the anus, the discomfort can lead to constipation.
Girls and young women who have this disease may experience some or all of the following symptoms in the vulvar area:

Lichen sclerosus is not a reported side effect of Arimidex.

The cause of lichen sclerosus is unknown.

Lichen sclerosus has been linked with low dihydrotestonerone, and my understanding of the way aromatase inhibitors (such as Arimidex) work suggests that there may be a sound hormonal basis for a link. I have just researched this (and hence found your blog) because I think I have lichen sclerosus (awaiting specialist opionion, but it fits clinically) and have been on Arimidex about a year. I am also a pathologist; whilst I am very familiar with the microscopic appearances in a vulval biopsy, I had no idea what a pain it was for the patient, or what caused it. Hope there are some effective therapies out there.

Thanks to Jan2011 for researching lichen sclerosis. After my initial posting here, I spoke to a Naturopath & she indicated that frequently lichen sclerosis is linked to extremely low estrogen levels (the goal of Arimidex).
Now, a year after seeing a dermatologist, and a prescription of betamethasone dipropionate ointment (0.05%) with weekly application, most of the itch is under control. The only down side appears to be the lifetime committment to the ointment.

I found this discussion in a google search, because I also suspect my 2 years on an AI (arimidex) in the development of my lichen sclerosis. Since stopping the AI (for reasons other than the lichen sclerosis), the symptoms and pain of the lichen sclerosis have abated but not entirely disappeared. I think this is one of those AI side effects that will take a long time to be recognized.
