You probably read that diltiazem should be used with caution or is contraindicated in Wolff-Parkinson-White syndrome.
Wolff-Parkinson-White syndrome is a syndrome of pre-excitation of the Ventricle of the heart due to an accessory pathway known as the bundle of Kent. Diltiazem may paradoxically increase ventricular rate in patients with WPW syndrome because of accessory conduction pathways.
