No, you don't need to take Premarin forever. If you are near the age of natural menopause (around age 50) and you are keen to stop talk with your doctor. You and your doctor could decide to stop taking Premarin and wait to see if you develop any menopausal symptoms. If you did develop symptoms and they were intolerable you could always take it again.
