How does a doctor decide whether to prescribe bitartrate or hydrochloride or propoxyphene?

It makes no difference.
Proproxyphene is the drug being prescribed. They will be equally as effective.

It is worth pointing out that on November 19, 2010 the US Food & Drug Administration requested that drugs containing propoxyphene be pulled off the market and banned in the United States because of a study linking propoxyphene to serious and sometimes fatal heart problems.
