It can stay for as long as 5 days on the receptor sites and block any other pain medicine, I have heard it can be found for up to 11 days, but, it has to be covering the receptor sites to block additional pain meds. You will know it is no longer filling the receptor sites if you are in moderate withdrawal. Moderate withdrawal means scoring 26 on the Clincal Opiate Withdrawal Scale. You can google that to find the sheet they use to guage the withdrawal symptoms.

Just wanted to give my input... I was addicted to opiates/Methadone for the past 2-3 years and just very recently decided that I "wanted" to stop for good. I got 2 8mg Subs and waited until I was in my 2nd full day of withdrawls to take half of the first one. I broke them up and used them over the course of 3 days and literally had the smallest most minor withdrawls... NOTHING compared to what I used to get from Methadone or Painpills... If you really "want" to quit use the Subs only to get rid of the withdrawls and for no more then 72 hours and u will be very pleased with the results... I tried once before this way but didn't wait until I was in withdrawls before taking the Subs and it sent me into the worst withdrawls of my life causing me to relapse... I've now been sober for 7 days and plan on staying that way for good... The withdrawls were honestly the thing that always made me relapse in the 1st place so Subs pretty much saved my life...
But again only if u aren't careless and stupid with them bcuz they too can cause addiction and withdrawls... Best of luck to all of u trying to quit!
