There is a generic Dilaudid (hydromorphone) tablet made by Roxane which is very close to the description you have given.

Pill imprint 54 196 4 has been identified as Hydromorphone Hydrochloride 4 mg.

Description: Hydromorphone Hydrochloride Tablets, USP, are supplied as white to off-white, round tablets. The 4 mg tablet is debossed with "4" on one side and "54 196" on the other side.
