... or can I treat with a change in diet and exercise?

You might have to just take Metformin. I was a bit overweight and my sugars were in the 170's and the doctor put me on Metformin and a diet. I lost weight and therefore my sugars dropped to normal levels. Doctor took me off Metformin and I have been diet and exercise controlled for about 3 years now! Make an appointment with your doctor and talk to them about Metformin. It may or may not be for you :).
