The hyrdromorphone is a slightly stronger opiate pain killer than the oxycodone 5/325. The apap stands for acetaminophen (tylenol) which should not exceed 1000mg every 6 hours for a total of 4000mg in 24 hours. The oxycodone 5/325 contains 325mg acetaminophen per pill. If she needs further pain relief I would start with taking two oxycodone every 4 hours. If that doesn't work wait 4 hours from last dose of oxycodone before trying the dilaudid (hydromorphone). The key with post op pain relief is to stay on top of it, before it gets bad, so taking the medicine on a regular schedule before pain gets unbearable is important.

I take 5mg oxycodone apap.
Instead of 2 every 4 hrs, she been taking one oxycodone every 2 hrs, and it does not last. This was to level load the medicine in the system, on the advice of her dr. The concern with exceeding this usage is the acetominophen, which as you note will exceed the limit of 4 grams per day. Hence the idea to switch to the dilaudid (no acetominophen). Thanks
